Training Contract/Agreement

Thank you for choosing Up Your Game for your personal soccer training! We look forward to working with you and taking that next step in your development. During your session, every effort possible will be made to ensure your safety. However, as with any physical activity, there is a heightened risk of injury. By voluntarily participating in this program and signing this form, you agree to the following terms and conditions:

• You agree to assume full responsibility for any injuries or personal damage that may occur during your session. 
• You acknowledge that no responsibility is assumed by the leaders of the training or the training facility in use.
• You acknowledge that a client canceling a private training session within 24 hours of the scheduled session time will result in forfeiture of the session and a loss of any financial investment at the rate of your chosen session.  
• You acknowledge that all clients arriving late will receive the remaining scheduled session time, unless other arrangements have been previously made with the trainer. 
• You agree to the possibility that pictures and/or film from your session could be used for our website or social media content
